We often receive precious tapes where we found no enclosure, handwriting was totally illegible, or critical contact information such as even an email address was omitted. In such cases, all we can do is await the "hate" call wondering why we haven't replied or acknowledged receipt of the materials. We're good at what we do...  just not clairvoyant nor experts at deciphering illegible handwriting !  If utilized, this form will resolve all those issues...

The info provided to us in this form will never be divulged or sold, nor will information ever become part of any mailing list.  Information collected is ONLY requested so we might have accurate contact information concerning your specific job.

General Instructions - PLEASE READ FIRST !

Failure to include information in the  purple colored boxes will result in the form not processing..   Enter 0 (zero) for postal code if none. If some of the fields are highlighted in yellow, you are probably running Google Toolbar, in which case you can have the toolbar's auto-fill function fill in those fields automatically.

2. You must provide a physical mailing address - UPS will not deliver to a PO Box

3. Clean record only applies to phonograph record transfers and audio restoration only to audio transfers.

4. Once properly filled out, left click and drag to select just the form. Then selectively print just the form itself.  Include a hard printed copy in with your shipment.

First Step:  Label your Media !

Label all your media with your last name, not just the cases, but also the media itself.  Unlabeled tapes are too easy to get mixed up with someone else's or separated.  If order is important, then also number them such as 1 of 3, 2 of 3 etc, as that will then be the sequence we will use in making the transfers.  Un numbered media will be transferred in random order or by our choosing !  A grease pencil usually works fine for this and is easily removed later if so desired, but any form of labeling is far better than none.

Content:  

You are responsible for sending us the correct tapes for transfer. If you do not know what is on them, then don't expect us to know either until after we make the transfer. 

(We're good at what we do....  just not clairvoyant !)  We do however offer screening services at additional cost to screen the tapes first for content. Unless screening is requested, we transfer all that is recoverable without regard to content - other than spot checking for copyright infringement.

* Screen Media for content

If you have a tape where you are uncertain of it's content, you can specify that we screen it first to get a rough idea of what it contains. We don't watch the entire tape, but rather take notes at approx 10 minute intervals down tape, and call you with what we find.  Since we only view for several seconds and then fast forward down tape, it's likely that we would miss desired content less than 10 minutes in length...    Cost for screening is dependent on the tape format as is published on our on-line price list.

Screening for content is far less expensive than having us make the transfer, only to find out that the tape did not contain what you thought it did..   The "down-side" is that the cost for screening applies whether or not we find what you are looking for...  If you are certain on what the tape contains, then screening is not warranted...  Note that if you thought the tape contained say a High School basketball game but instead the content was all of the Glee Club, unless you specified for us to screen the tape first, we transfer the entire tape regardless of content...   We do not check to see if further down tape was where the basketball material might be located  --  Without screening, we just transfer everything assuming the basketball game was further down tape....

** Clean Record - Audio Restoration

NO is the default value for both these fields.  Change to YES if you desire either cleaning or restoration

Pertains only for phonograph and Gramophone record transfers.  Your record is professionally cleaned using the wet vacuum system.  Some vintage 78's cannot be wet cleaned such as Recordio discs with an acetate surface over a fiber substrate.

Audio restoration involves removal of rumble, upper end surface noise and reduction or elimination of impulse noise such as scratches, pops and clicks and some background noise depending on it's frequency distribution.  Otherwise, the transfer is considered a "needle drop" and is the audio that came directly off the turntable.

Shipping

We strongly suggest you ship via UPS or FedEx...  Please DO NOT use the US Postal Service unless you ship via Express or Priority... do not use First Class or Media Mail.   Confirmation of delivery by the USPS only means it was delivered somewhere and not necessarily to the correct address.  On average, twice each week, we receive someone else's mail. It's not uncommon for packages addressed here to be delivered to our neighbors or somewhere else in town...  With their poor tracking, we strongly suggest you do not ship us anything via the US Postal Service...  With UPS or FedEx, there is never a problem...   (Course, that's why they even exist in the first place !)
